COMPLAINT: neck and back pain.

SUBJECTIVE:pt was driving and hit a car. pt does not remember accident. air bags did not deploy.

OBJECTIVE: pt has a history of high bp and a diabetic. pt had abrasion to the left eye, hematoma on the left elbow, right middle figure pain, abrasion to the left elbow. nkda. pt has history of high bp and diabetes. compliant with both meds. no crepidus in right middle finger. transported to grady

TREATMENT:12 lead acquired, took vitals, full spinal immobilization, 22g left hand. stopped bleeding above the left eye and left forearm.
