COMPLAINT: pt vomiting blood for 3 days

SUBJECTIVE:pt stated he had been coughing up blood for 3 days.

OBJECTIVE: a.o.s to find pt sitting on a couch in his apartment. pt has a history of copd, htn, diabetes. pt has been coughing up blood for 3 days. pt takes multiple medications and nkda.

TREATMENT:took vitals, 20 g in left ac, 12 lead acquired. pt on 3 lpm of 02. transported to grady
