AGE:  65 YEARS

REASON WHY YOU WERE DISPATCHED:Vertigo/ Nausea/ Dizziness

COMPLAINT: Vertigo, Nausea, Dizzness

SUBJECTIVE:Patient said the dizziness, headache, and vommiting started about 730 am this morning. Pain is a dull pain in her RUQ. 5 out 0f 10 on a pain scale. When patient puts her head down she feels like she is going to vomit.

OBJECTIVE: Patient came into triage and sat down. Patient was shivering. AOx3. History of vertigo and breast cancer.

TREATMENT:Took patients vitals.
